Requirements for nonprofit hospitals to conduct periodic community health needs assessments, to engage public health partners in community health planning, and to report their contributions to their communities all contribute to an environment conducive to collaboration.In this LearnScape, the student is functioning as an external consultant working with the Director of Marketing, at Bright Road Healthcare System, to find ways to improve the functionality and utility of the system’s patient health record (PHR) application. Population health management focuses on promoting wellness through an emphasis on prevention which in turn controls healthcare costs. This should result in a reduction in the emergency room visits, which is perhaps one of the most expensive points of service in the U. S. healthcare delivery system. In order to make a comprehensive population health management initiative successful, it requires a high degree of collaboration between chief stakeholders (public health, hospitals, academic medical centers, insurance/managed care organizations, patient-centered medical homes, accountable care organizations, and, of course, the population). There is also a need to have adequate information systems to support such a comprehensive wellness promotion strategy. The personal health record (PHR) is one such system that is designed to permit patients to compile and maintain information about their health. In addition, clients retain control over how the health information is accessed, used, and disclosed to third parties. While the PHR is similar to an official medical record, they are not the same. The medical record contains information provided by healthcare personnel. The PHR and patient portals are examples of information systems that are designed to increase population health management, foster patient engagement, and promote repeat business to healthcare organizations and providers.
